Brief Description

The Social Services Coordinator at Inglis House is responsible for securing/supporting/coordinating behavioral health services and psychosocial support for residents. Key duties include providing concrete services, evaluating mood, monitoring behavior, implementing appropriate interventions, serving as an advocate for residents, and coordinating with families and other entities.

Essential Functions

  • Ensure transportation applications are filled out for residents within the indicated deadline.
  • Assess and respond to residents' and/or family’s requests promptly; return phone calls within 24-48 business hours.
  • Facilitate and resolve concerns through the concern process, referring them to appropriate staff for resolution.
  • Support grievance investigations and communicate resolutions to residents and/or their representatives within specified timelines.
  • Aid in discharge planning as needed and requested by the Director of Social Services.
  • Assist with additional tasks and responsibilities as directed by the Director of Social Services and Nursing Home Administration.
  • Develop a comprehensive social history and psychosocial assessment, involving residents, families, and representatives in care planning.
  • Provide a consistent forum for residents to express their needs, concerns, and wishes, and attend care conferences to interpret psychosocial needs and include them in the care plan.
  • Document findings of reported allegations of abuse and/or neglect within 24 hours of discovery, report to the Nursing Home Administrator, and review resident rights annually.
  • Complete progress notes for behavioral concerns within one business day, MDS assessments within five business days, and Social Work assessments within fourteen days of admission.
  • Update, revise, and adjust residents’ care plans based on MDS assessments, witnessed and reported behaviors, psychotropic medications, discharge disposition, and identified psychosocial and behavioral health supports.
  • Develop and implement person-centered interventions to meet residents’ needs, consult with interdisciplinary teams, and attend Gradual Dose Reduction Meetings.
  • Complete quarterly assessments and notes, and consult with the Director of Social Work, counselors, and Psychiatrist to develop behavioral plans and contracts as needed.
  • Implement person-centered interventions to residents of Inglis House, consulting with residents, support systems, and interdisciplinary teams.

Qualifications

  • Minimum of a bachelor’s degree in social work or a related field (e.g., sociology, gerontology, special education, rehabilitation counseling, psychology).
  • One year of supervised social work experience in a healthcare setting working directly with individuals.
  • Ability to function independently, work effectively with residents, staff, and support agencies, and relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people.
  • Ability to make independent decisions when circumstances warrant such action, and possess excellent listening and communication skills.

Pay

$55,227 - $72,220 Annually
