Social Services Caseworker
Purpose Statement/Position Summary
The Social Services Caseworker position aims to improve, reduce, and eliminate family barriers while advocating, supporting, providing guidance, and coordinating access to care. The Caseworker provides direct care, collaborates with families, multidisciplinary staff, and submits timely documentation to promote children’s social, emotional, and developmental growth. The Caseworker will collaborate with our interdisciplinary Community Behavioral Health team and is responsible for following up with clients/families to assist with community resources such as: Special Education, Regional Center, ABA, Legal Services, Conservatorship, Housing, Transportation, Immigration, Mental Health referrals, Food Banks, and obtaining documentation from community agencies etc. The caseworker is an essential part of a client’s treatment, as it creates opportunities to integrate families into their local community, strengthen partnership with local organizations and increase awareness of available resources.
Required Minimum Qualifications/Work Experience
- 1+ years of social work/case management experience in a pediatric medical setting or an equivalent combination of related education and work experience.
- Lived experience as a parent or caregiver of a child who was hospitalized during infancy and/or received early intervention services during that period.
- Must be willing and able to work fully onsite.
Preferred
- Bilingual in English/Spanish.
- Knowledge of neurodevelopmental and related disabilities.
- Experience with County and Public Social Services Departments.
Education/Licensure/Certifications
- Bachelor’s degree in Social Work or an equivalent combination of related education and work experience.
- American Heart Association BLS certification required.
