Job Summary

The Social Services Assistant supports the Social Services Department in meeting the psychosocial needs of residents in a skilled nursing facility serving veterans. This role involves assisting with admissions, care planning, discharge planning, and daily social work functions to promote residents' emotional well-being and maintain or improve their highest practicable level of functioning.

Essential Functions

  • Aid residents and families with psychosocial support, adjustment to facility living, and emotional concerns.
  • Support the Social Services Director in developing and implementing individualized care plans.
  • Participate in interdisciplinary care plan meetings, assessments, and case reviews.
  • Coordinate admissions and discharges, including education for families and community resources.
  • Maintain documentation in accordance with federal, state, and facility policies (e.g., MDS 3.0 Section Q, progress notes).
  • Advocate for the rights of residents and help address grievances or concerns in a timely and empathetic manner.
  • Provide information and referrals to community resources, VA benefits, mental health services, and other support services.
  • Support and help implement activities or programs designed to enhance residents' quality of life.
  • Promote dignity, autonomy, and cultural sensitivity in all interactions with residents and families.
  • Comply with all HIPAA regulations and resident confidentiality standards.

Required Skills

  • Experience: Minimum of one (1) year experience in a healthcare setting preferred; long-term care or veterans care experience is highly desirable.
  • Licensure: Social Work licensure in the state of Georgia is preferred but not required for this position.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Excellent organizational and time management skills.
  • Career-oriented and resident-centered approach.

Required Education

  • Education: Bachelor’s degree in Social Work, Psychology, Sociology, or related human services field is required.

Working Conditions/Physical Requirements

  • Primarily works in an indoor healthcare environment.
  • Occasional lifting, walking, and assisting with resident interaction.
  • Must be able to manage stress and maintain a professional demeanor in emotionally charged situations.
