Social Services Assistant
About the role
At Shalom Gardens Health and Rehab, we are dedicated to providing compassionate, resident-centered care. We believe that meeting the emotional, social, and psychological needs of our residents is just as important as their medical care. Our Social Services team plays a key role in ensuring our residents and families feel supported, valued, and understood.
Key Responsibilities
- Assist with resident assessments, care plans, and documentation
- Provide support during admission, transfer, and discharge processes
- Help residents and families adjust to long-term care placement
- Advocate for residents’ rights and quality of life
- Facilitate communication between residents, families, and the interdisciplinary care team
- Assist in organizing and implementing programs that support residents’ social and emotional well-being
- Maintain accurate records and documentation
Qualifications
- High school diploma or equivalent (Associate’s or Bachelor’s in Social Work, Psychology, or Human Services preferred)
- Experience in long-term care, healthcare, or social services field preferred
- Strong communication, organizational, and interpersonal skills
- Caregiving, patient, and resident-focused approach
- Ability to maintain confidentiality and professionalism
