Responsibilities
- Collections and assessment of data relevant to patients' psychosocial needs, risk factors for psychosocial deterioration and responses to interventions.
- Implementation of social service interventions that achieve treatment goals, address resident needs, link social support, physical care and physical environment to enhance quality of life.
- Completion of required forms and documentation in accordance with company policy and state and/or federal regulations.
- Coordination of discharge planning.
- Facilitation of advanced directive decision-making process.
- Coordination of Family council.
- Coordination of the grievance complaint process.
- If appointed Grievance Official, responsibility for overseeing the grievance process.
Qualifications
- Bachelor's degree or above in Social Work (BSW) or related field required, such as Human Services, Psychology, Sociology, Gerontology, Special Education, and/or Rehabilitation Counseling.
- Social Worker license in the state of employment a plus / required (i.e., LBSW, LSW, LCSW, LMSW, etc.).
- One (1+) years supervised social worker experience working directly with elderly individuals in a health care setting required.
- Prior Long-Term Care, Skilled Nursing, or related healthcare social work experience strongly preferred.
