Social Service Designee

About the role

We are hiring a Social Services Designee to assist residents and their families in adjusting to placement, illness, and care plans, ensuring the highest level of functioning in compliance with federal and state regulations. Under the supervision of the Social Services Director, you will support residents' transition, participate in resident and family council meetings, and aid in discharge planning and admissions.

Responsibilities

• Helping residents and families with treatment wishes and adjustments
• Supporting care plan implementation and compliance
• Assisting with discharge planning and admissions processes
• Participating in meetings and providing emotional support

Qualifications

• High school diploma or equivalent
• Ability to work with elderly, mentally or physically ill, and emotionally upset individuals
• Strong organizational, prioritization, and confidentiality skills
• Knowledge of long-term care laws, regulations, and nursing procedures

Preferred Skills

• Experience with geriatric populations
• AA or Bachelor’s degree in a related field
• Creative problem-solving and strong communication skills
• Computer proficiency for record management
