About the Role
A Social Services Coordinator reviews, monitors, and processes assigned client cases involving social, health, vocational, rehabilitation, or related services to City residents. This role involves no supervisory duties and receives general supervision from a technical superior. Work is performed in various settings, including homes, health clinics, and neighborhood communities, with possible exposure to challenging conditions. The role requires minimal physical exertion and follows a conventional workweek.
Responsibilities
- Review, monitor, and process client cases involving health, social, vocational, or rehabilitation services.
- Coordinate and evaluate the implementation and progress of recommended care or strategies to ensure compliance with professional standards.
- Maintain confidential, accurate, and detailed client case records.
- Conduct and process multiple case assignments simultaneously.
- Motivate clients to follow designated regimens or courses of action to effect behavior modifications.
- Coordinate responses from multiple public and private agencies, institutions, and community resource organizations.
- Collect, review, and analyze client case records to recommend improvements in reporting and documentation practices.
- Disseminate information and explain City, State, and private institutional services and programs to individuals and groups.
- Establish and maintain effective liaisons with clients, families, government representatives, and service providers.
- Handle emergency or crisis situations with calm, tact, and efficiency.
Requirements
On or before the date of filing the application, each candidate must meet one of the following:
- Education: A bachelor’s degree from an accredited college or university in vocational rehabilitation, health or human services, nursing, or a closely related field.
- Experience: One year of experience in coordinating, monitoring, and processing cases involving health, social, vocational, or rehabilitation services.
- Equivalency: An equivalent combination of education and experience.

- Knowledge of public and private community, social service, and health agencies, including their functions, resources, and services.
- Knowledge of individual, age-specific, and community needs, as well as available resources.
- Knowledge of social, public health, medical, rehabilitation, and educational resources.
- Knowledge of interview and investigation techniques.
- Skill in maintaining confidential and accurate client case records.
- Skill in managing multiple case assignments simultaneously.
- Skill in motivating clients to modify behavior.
- Skill in analyzing client records to evaluate care or treatment progress.
- Skill in coordinating responses from multiple agencies and organizations.
- Skill in simplifying reporting and documentation practices.
- Ability to communicate effectively, both orally and in writing.
- Ability to explain services and programs to diverse audiences.
- Ability to establish and maintain effective liaisons with varied backgrounds.
- Ability to handle emergency or crisis situations calmly and tactfully.
- Proficiency in operating standard business PC equipment and software.

Pay
Salary Range: $48,653.00 - $79,299.00 annually
Hiring Salary Range: $48,653.00 – $63,976.00 annually
Additional Information:
- Eligible candidates under final consideration will undergo a criminal background check and/or fingerprinting.
- A six-month probationary period is required for all selected candidates, including current City employees.
- Qualified candidates will be considered for vacancies for at least six months.
- Education must be from an accredited institution; foreign credentials must be evaluated for U.S. equivalency.
