About the role
Eskaton is a nonprofit organization dedicated to supporting older adults and those who care for them. Founded over 55 years ago, we operate 23 communities across Northern California, fostering vibrant, supported, and celebrated aging experiences.
Responsibilities
- Provides general case management (including intake) and referral services to all residents needing such assistance.
- Makes formal case management evaluations of health, psychological, and social needs, develops individualized case plans for services, and periodically reassesses residents' situations and needs.
- Establishes connections with relevant community agencies and service providers, negotiating the best deals for personalized, flexible, and innovative services.
- Develops a directory of service providers for both project staff and residents, facilitates referrals, and educates residents about service availability, application procedures, and resident rights.
- Monitors the delivery of services from community agencies and updates the case management and provider agency on the resident's progress.
- Manages supportive services where applicable, assists residents in building informal support networks, and may set up volunteer support programs.
- Trains project residents on their obligations of tenancy or coordinates such training, and educates staff on aging in place and service coordination.
- Develops case plans in collaboration with community assessment services or a PAC (Professional Assessment Committee).
- Maintains accurate records and files in a confidential manner, accessible only by the Executive Director, Executive Director, or CEO.
- Completes other duties as assigned by the Executive Director, excluding providing social activities, assessing residents' level of functioning, providing transportation, providing medical services or advice, providing direct services or care, or performing office/clerical tasks.
Qualifications
- Education: A Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling is preferred; a college degree is acceptable.
- Experience: Two years of progressive responsibility in management functions, with demonstrated knowledge of supportive services and resources for senior citizens and non-elderly disabled.
