Social Care Coordinator - Hybrid

About the role

Epicured is hiring a Social Care Coordinator to support New York’s Medicaid Section 1115 Waiver program. In this role, you’ll schedule and support non-clinical assessments, navigate members to medically tailored nutrition services and other Health-Related Social Needs (HRSN) supports, and coordinate referrals across internal teams, community-based organizations (CBOs), and Social Care Network (SCN) partners. This role, reporting to the VP of Member Experience, is ideal for someone who is organized, detail-oriented, and comfortable managing member communications and compliant documentation in a fast-paced care coordination environment.

Responsibilities

• Complete training for total access to SCN Platforms
• Perform operating tasks – Member Intake into Epicured Management System (EMS)
• Schedule and support completion of non-clinical assessments and ensure timely follow-up
• Coordinate referrals across internal teams and external community-based organizations (CBOs) and Social Care Network (SCN) partners
• Provide member support on Inbound and Outbound contacts leveraging Five9 and other partner technology platforms
• Provide ongoing care management and engagement support to active members throughout their program participation
• Support closed-loop referral processes, including tracking, follow-up, and confirmation of service delivery
• Respond to member inquiries related to services, program participation, and billing coordination in a timely and professional manner
• Adhere to all applicable Medicaid, HIPAA, and NYS DOH privacy, compliance, and documentation standards

Requirements

• 3–5 years of customer service or member engagement experience
• Knowledge of Medicaid and/or HRSN programs
• Strong communication skills
• CRM or care management platform proficiency
• Problem-solving and active listening skills

Qualifications

• Background in healthcare, care coordination, patient support, or community-based services (preferred)
• Bilingual English/Spanish or multilingual candidates encouraged to apply (Spanish, French, Mandarin, Hindi)

$21–$23/hour, based on experience
