Services Coordinator
Job Summary
The Services Coordinator at Booth Manor, a division of The Salvation Army North & Central Illinois Division, assists and advises residents and families on services necessary for a self-reliant lifestyle. They promote wellness activities, educate residents on available services, and act as a liaison between community agencies and residents. This role requires collaboration with the property manager and other management staff.
Essential Duties And Responsibilities
- Affords residents and families advice on necessary services to maintain a self-reliant lifestyle.
- Promotes wellness activities for all residents.
- Educates residents, families, and staff on available community resources.
- Builds informal support networks among residents and families.
- Liaises with community agencies, service providers, and residents.
- Serves as a team member with the facility manager and other Booth Manor staff.
- Encourages residents to take proactive steps in meeting their social, psychological, and physical needs.
- Facilitates meeting of needs when necessary, avoiding unhealthy dependence.
- Uses the least drastic intervention necessary to alleviate problem situations.
- Makes volunteer support programs with service organizations available to residents.
- Advocates and negotiates on behalf of residents for adequate, timely, and cost-effective services.
- Monitors the delivery of services to ensure they are appropriate, timely, and satisfactory.
- Provides limited case management for residents, including evaluating social, psychological, and physical needs and developing a service plan.
- Reports all suspected abuse situations to the appropriate agency.
- Assembles a directory of community services and makes it available to residents, families, and management.
- Aids management in identifying residents who need assistance.
- Maintains individual files on residents containing intake information, service termination information, quarterly reviews, abuse records, and resident/family meeting notes.
- Ensures confidentiality of resident files by keeping them in a secure area.
- Completes reports with copies given to the site manager and HUD officials upon request.
- Pursues additional services through private, local, state, and federal sources.
- Creates a service management plan as needed.
Minimum Knowledge, Skills, And Abilities
- Bachelor's degree required, preferably in Social Work, Gerontology, Psychology, or Public Health.
- 2-3 years of experience in Social Work or a related field.
- Demonstrated working knowledge of community services in the region, particularly those provided for Booth Manor residents.
- Proven experience in service management, including organizing, problem-solving, and advocating.
- Knowledge of the aging process, elder services, disability services, drug and alcohol abuse, and mental health issues.
- Awareness of eligibility and procedures for federal and state entitlement programs.
- Understanding of legal liability issues related to service coordination.
Reporting Relationships
This position reports to the Assistant Divisional Director of Social Services. The individual acts as a representative of The Salvation Army and its mission in all contacts related to this position.

Physical Demands/Work Environment
- Required to do moderate physical work and lift up to 10 pounds.
- Works in an office environment with a low noise level.
