What You’ll Do
- Meet with families to review needs, goals, and available supports
- Help families access services and community resources
- Cook up referrals and communication with service providers
- Aid in Individualized Family Service Plans (IFSPs)
- Schedule and lead IFSP team meetings
- Support families in advocating for their child’s needs
- Help identify service gaps and assist with problem-solving
- Maintain accurate records and follow DHHS guidance, policies, and regulations
What We’re Looking For
- Qualified candidates must have one of the following:
- Two years of experience in a health or human services field
- 24 semester hours of education in behavioral sciences, social sciences, or a related field
- A combination of education and experience equal to two years in a related field

- Knowledge of child development
- Strong communication and organization skills
- Ability to work with families, providers, and community partners
- Respect for confidentiality
- Ability to work with diverse families
- Microsoft Office skills
Preferred, But Not Required
- Experience working with infants, toddlers, or children with special needs
- Experience with family-centered services
- Knowledge of community agencies and developmental disabilities
- Bilingual skills
