About the role

The Service Coordinator (Mental Health Case Manager) at Crestwood is a key member of the interdisciplinary treatment team. This role involves assessing clients, coordinating team meetings, and implementing treatment plans.

Responsibilities

  • Assess clients to identify problem areas requiring treatment and complete assessments.
  • Coordinate interdisciplinary team meetings and discharge planning efforts.
  • Work closely with nursing staff to ensure continuity of treatment efforts.

Qualifications

  • Two (2) years of full-time experience or part-time equivalent in a mental health setting.
  • A bachelor’s degree in psychology, social work, or behavioral sciences may substitute for the experience requirement.
  • Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
  • Familiarity with regulatory standards.

Pay

The salary range for the Service Coordinator (Mental Health Case Manager) is $25 USD - $27 USD per hour, with final salary determined by the candidate's qualifications and program acuity.
