Service Coordinator | Mental Health Case Manager
Job Duties
- Assesses clients to identify individual problem areas necessitating treatment and complete assessments.
- Coordinates interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
- Collaborates with the nursing staff to ensure continuity of treatment efforts for the client on the unit.
Qualifications
- Two (2) years of full-time experience or part-time equivalent in a mental health setting.
- A bachelor’s degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
- Familiarity with regulatory standards.

About the Campus
The new Healing Center, serving the county through our Psychiatric Health Facility (PHF), will be located in the beautiful and historic city of San Luis Obispo. This addition to the Crestwood Family will be a 16-bed campus, marking a significant step in our commitment to supporting and developing our relationships with the community.
Pay
The salary range for this position is $24.20 USD - $25.68 USD. Final salary offered will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.
