Responsibilities

  • Help people with developmental disabilities and their families identify and obtain needed services and supports.
  • Advocate for, monitor, and evaluate services along with the individuals, their families, or guardians.
  • Manage a caseload of varying ages, disabilities, and case complexity to improve the quality of support and services by establishing strong, trusting relationships through regular contact and planning with the individual and their team to identify needs and create an action plan.
  • Develop community relationships and support systems; link resources by matching the unique support needs of individuals and families with community resources. This may involve researching existing resources, developing new resources, making referrals, preparing Utilization Review requests, service authorizations, and coordinating federal, state, county, and community programs.
  • Ensure quality enhancement by monitoring and assessing the quality, timeliness, and effectiveness of services and support through an ongoing review process (via phone, email, or in-person meetings). In-person meetings occur monthly, quarterly, and annually in settings such as individuals’ homes, day programs, community settings, offices, group homes, or residential settings.
  • Follow reporting processes for findings from monitoring, including protocols for mandated reporting in cases of suspected abuse and neglect.
  • Complete documentation via written case notes for all activities within three business days of TCM activity, maintaining a minimum of 120 TCM billable hours per month.
  • Provide up-to-date information to people supported by participating in knowledge enrichment activities such as training, continuing education, and attending conferences or seminars.
  • Create team unity by accepting and willingly carrying out special assignments or duties.

Skills and Abilities

  • Ability to work with a variety of professionals from different disciplines to integrate their evaluations into the client’s individual support plan.

  • Ability to work professionally with individuals of economically and culturally diverse backgrounds, establishing clear communication with persons receiving services, families, service providers, and outside agencies.
  • Ability to complete work accurately within specified timelines and return calls/emails within one business day.
  • Ability to set, communicate, and follow timelines while maintaining adaptability for unforeseen changes.
  • Ability to work independently, be resourceful, and ask for assistance when necessary.
  • Ability to apply mathematical skills and concepts to understand, review, create, and maintain budgets according to the needs of individuals served.

  • Ability to facilitate and lead an ISP (Individualized Support Plan) team.

Qualifications

Required Education: A Bachelor’s degree from an accredited college or university or a Registered Nurse license.

  • Must have reliable transportation and an acceptable driving record.
  • Must use your own vehicle for work-related transportation.
  • Must be able to drive for work-related assignments to individuals’ homes to meet face-to-face with individuals on caseloads.
  • Must have a valid driver’s license (a Class E Missouri Driver’s license is preferred but not required).
  • Must provide proof of personal, valid vehicle insurance.
  • Must have access to reliable internet for telecommuting employees.

Preferred Experience:

  • Experience working with individuals with intellectual and/or developmental disabilities in a community-based setting as a Registered Nurse, or in social work, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, or a closely related area.
  • Experience with case management and documentation applications such as Setworks, Kronos, Therap, etc.
  • Experience as a member of a multi-disciplinary team in assessing, developing, or implementing individual person-centered plans, especially for persons with intellectual/developmental disabilities.
  • Experience utilizing the Microsoft Office Suite.

Schedule

Full-time (40 hours per week), Monday through Friday, 8:00 AM to 5:00 PM.
