Essential Duties and Responsibilities
- Captures the ongoing Individual Service Plan (ISP) process, including thorough assessments, coordination of ISP team meetings, development of comprehensive ISPs, ongoing reviews to ensure services and supports are provided in accordance with the ISP, verification that the ISP ensures health, safety, and welfare, reflects current levels of needs and services based on assessment results, and aligns with individuals' preferences.
- Coordinates ISP meetings in alignment with self-determination principles, plans services to meet assessed needs through a unified ISP, develops ISPs that support self-determination concepts and principles as defined by Ohio Department of Developmental Disabilities rules and MCBDD policies and procedures, including individuals' choices of service providers, development of personal budgets, and individual control of services.
- Writes ISPs within SSA department guidelines, ensuring they are distributed to the team before service delivery dates. Communicates service dates, corrections, suspensions, disenrollment to Waiver Administration, coordinates changes, ISP revisions/reviews, and preventive measures as necessary.
- Complies with local, state, federal rules, regulations, statutes, and standards, including DODD and CMS interpretations.
- Supports and encourages active participation by individuals and significant others in the planning process.
- Recommends on the ISP team or for ICFs (as assigned) to facilitate the development of a unified community plan of service.
- Acts as the primary coordinator for individuals and between team members for the ISP, attends meetings, hearings, etc., at various sites, assists individuals/families in exercising their rights, provides feedback on the service review process, and assists individuals in obtaining appropriate services.
- Promotes the involvement of agencies and individuals to arrange services impacting the individual's life, eliminates duplication, and maintains records and reports in a timely and accurate manner, consistent with accreditation and regulatory standards or contracted agreements.
- Maintains contact with individuals and all ISP team members, maintains effective working relationships with staff of other agencies and facilities, families, and the public.
- May arrange or provide transportation for individuals, provides consultation to staff on specific cases.
- Documents daily and submits monthly Medicaid Waiver billing documentation in accordance with the Targeted Case Management and/or Medicaid Administrative Claiming Guide, attends required training, and identifies any training needs, requesting support to ensure complete and accurate documentation.
Qualifications
- Bachelor’s degree (B.A.) from four-year college or university required.
- To perform successfully, knowledge of database software, spreadsheet software, internal network functions, and word processing software is required.
- Must meet eligibility requirements for DODD Service and Support Certification.
- Valid State of Ohio driver’s license required, meeting agency driving requirements.
- Must be available to work outside normal program hours, on a rotating “on-call” basis, and have a varied and flexible schedule to meet the needs of assigned individuals. Must be able to travel to various community-based sites or programs. Not all sites/environments are accessible for individuals with mobility issues.
- Must meet all other agency employment requirements.
- Must meet all requirements as outlined on the attached job description.
Benefits
At least $48,000 annually; to commensurate with education and experience.
