About the Role

The Senior Case Manager is the subject matter expert for the case management team, responsible for mentoring the team and ensuring all delivered services meet the needs of SSVF clients. This role combines administrative/office duties with field/outreach work, requiring on-site presence daily.

Responsibilities

  • Assign and monitor case load of the team, including reviewing daily referrals and screenings, designating clients to case managers, and verifying supporting documents.
  • Provide information and referrals for callers and walk-ins.
  • Conduct comprehensive screenings and assessments to develop case plans.
  • Develop support systems, coordinate move-in activities, rental negotiations, and service referrals.
  • Make home and field visits as needed.
  • Monitor case managers regularly to ensure quality and effectiveness of services.
  • Review cases with the Program Coordinator and Case Management Team.
  • Audit case files for completeness and accuracy.
  • Conduct crisis intervention as necessary.
  • Assist case managers in identifying and coordinating services for veterans and families.
  • Make referrals to VA/community providers for therapeutic, social, educational, and medical resources.
  • Provide support through case conferencing, counseling, and advocacy.
  • Conduct comprehensive file reviews to ensure program services eligibility.
  • Maintain secure program assets and client records.
  • Develop and assist with workshops pertaining to program services.
  • Participate in outreach events.

Qualifications

  • Education: Bachelor’s Degree in Social Work or related field, or a combination of education and relevant experience (four years’ total). Completion of VOALA Case Management training within 6 months of hire.
  • Experience: Minimum 3 years as a case manager.
  • Preferred Qualifications:
    • Master’s Degree in Social Work or related field.
    • 4 years of experience working with the target population.
    • Experience in a supervision or lead role.
    • HMIS navigation experience.

Requirements

  • Ability to pass a fingerprint clearance, background check, including criminal history, personal references, employment, and education verifications.
  • Ability to perform under circumstances of possible emotional stress and conflicts, dealing with difficult, uncooperative, and potentially aggressive clients.
  • Mandated Reporter status, requiring reporting of any physical or sexual abuse to proper authorities.

Pay

$26.68 - $28.08 Per Hour
