Senior Case Manager
Dungarvin is a national organization of privately owned companies dedicated to providing high-quality, community-based support to people with intellectual and developmental disabilities, individuals facing mental health, behavioral, or significant medical challenges, and children and their families.
About the role
The senior case manager position is designed to assist persons in identifying and acquiring supports that allow them to live in the community. The senior case manager supports individuals in making their own choices and decisions about these supports, functioning as an educator and resource to both clients and peers within the department. The role focuses on coordinating services for vulnerable individuals, helping them navigate complex service delivery systems through ongoing assessment, planning, referral, service coordination, monitoring, consultation, and advocacy. This position directly manages a caseload and facilitates other services or supports to positively influence individuals’ lives. The senior case manager will engage with a wide range of organizations, families, and community members, representing both the interests of the individuals they support and the case management department.
Responsibilities
- Assist individuals in identifying and acquiring community-based supports.
- Support individuals in making their own choices and decisions about services.
- Function as an educator and resource to clients and peers within the department.
- Coordinate services for vulnerable individuals, helping them navigate complex service delivery systems.
- Conduct ongoing assessment, planning, referral, service coordination, monitoring, consultation, and advocacy.
- Manage a caseload and facilitate or coordinate additional services or supports.
- Maintain professional contact with organizations, families, and community members.
- Represent the interests of both the individuals supported and the case management department.
Requirements
- Bachelor’s Degree in a human service-related field.
- Two years of experience working with individuals with disabilities or aging populations.
- Two years of full-time experience in CADI/DD waiver case management.
- Knowledge of the local community and concepts related to self-directed services.
- Good oral and written communication skills.
- Valid driver’s license and proof of automobile insurance.
People employed in this position are expected to meet the qualifications of an unlimited driver, defined as requiring an employee to drive a motor vehicle as part of their work, including Dungarvin-owned or leased vehicles, or transporting individuals served.

Pay
Starting salary: $55,100.00 - $56,615.25 annually. The listed salary range is based on experience level and may vary depending on an individual’s skills and qualifications.
