Senior Case Manager
Duties And Responsibilities
Conduct comprehensive biopsychosocial, vocational, and strengths-based assessments to determine participant eligibility, service needs, and employment readiness.
Complete intake interviews, enrollment documentation, assessments, consent forms, and required program paperwork.
Develop, implement, and monitor individualized service plans that identify participant goals, barriers, interventions, and measurable outcomes.
Maintain a caseload of participants with higher social service, behavioral health, and workforce development needs.
Provide ongoing individual coaching focused on employment readiness, career development, self-sufficiency, and personal growth.
Utilize trauma-informed and evidence-based approaches, including Motivational Interviewing, Strengths-Based Practice, Harm Reduction, and Solution-Focused interventions.
Conduct regular participant meetings to monitor attendance, engagement, internship performance, and progress toward goals.
Support participants in addressing barriers related to housing instability, financial hardship, transportation, childcare, mental health, substance use, legal involvement, and other social determinants of success.
Collaborate with interdisciplinary team members and partners to assess and identify resources needed to support participants' ability to achieve program completion.
Develop and facilitate workshops, support groups, and psychoeducational sessions related to workforce readiness, wellness, professional development, and life skills.
Pay Rate
$65k-$70k based on education, experience/skillset
Partnership Development
Connect participants to internal and external resources that support stability and long-term success.
Facilitate referrals and warm handoffs to community-based providers, including housing, healthcare, behavioral health, childcare, legal services, educational opportunities, public benefits, food assistance programs, and other social service needs.
Advocate on behalf of participants to ensure equitable access to services, supports, and opportunities.
Staff Supervision and Professional Development
Provide supervision, mentorship, coaching, and professional development to Case Managers and MSW interns.
Conduct individual and group supervision sessions focused on participant engagement, documentation quality, case management practice, and professional growth.
Lead case conferences and multidisciplinary team meetings to review participant progress and address complex cases.
Provide guidance regarding crisis intervention, de-escalation, mandated reporting, ethical decision-making, and participant safety concerns.
Support onboarding, training, and professional development activities for staff and interns.
Foster a culture of accountability, continuous learning, reflective practice, and participant-centered service delivery.
Model ethical practice, professional boundaries, effective communication, and collaborative leadership.
Compliance and Data Management
Ensure timely, accurate, and comprehensive documentation in all required databases and case management systems.
Monitor participant engagement, attendance, retention, completion, credential attainment, internship participation, and employment outcomes.
Prepare reports, participant success stories, case summaries, and outcome analyses as required.
Identify trends, service gaps, and barriers impacting participant success and recommend corrective actions.
Utilize data to inform service planning, program improvements, and strategic decision-making.
Collaborate with program leadership to implement strategies that improve participant outcomes and program performance.
Crisis Intervention and Risk Management
Respond to participant crises involving mental health concerns, homelessness, domestic violence, substance use, community violence, or other emergencies.
Conduct risk assessments and safety planning when appropriate.
Coordinate crisis response services, referrals, and follow-up support while ensuring participant safety and continuity of care.
Collaborate with program leadership and external providers regarding high-risk situations and emergency interventions.
Maintain accurate documentation and reporting related to crisis incidents.
Program Development
Identify emerging participant needs and recommend innovative service strategies and program enhancements.
Assist in the development of workshops, curricula, participant engagement initiatives, and retention strategies.
Participate in program planning, strategic initiatives, and cross-departmental projects.
Support the integration of social work best practices into workforce development programming.
Contribute to a client-centered culture that promotes equity, inclusion, wellness, and long-term success.
