About the role

JRI's Program RISE is a multi-service component of JRI Health, serving the Metrowest area (Framingham, Natick, Marlborough and surrounding towns). Our program offers preventive health and social services, such as confidential screening for HIV, Sexually Transmitted Infections (STIs), and viral Hepatitis; HIV client services, such as medical and housing case management and group support services for individuals affected and living with HIV/AIDS.

Responsibilities

  • Provide direct case management services to clients.
  • Assess client eligibility for the program and triage incoming client cases.
  • Provide leadership, guidance, and administrative direction to HIV case management team.
  • Oversee day-to-day operations of the HIV case management programs.
  • Participate in program and division wide meetings as required.
  • Understand and be able to apply practices that align with Program RISE service delivery.
  • Create a strong connection with individuals served, staff, and other service providers while maintaining professional boundaries.
  • Complete all documentation in a timely manner (i.e., by program, division, and funder deadlines) according to policies and procedures.
  • Ensure effective implementation of current and new policies and procedures set forth by JRI, JRI Health and its funders.
  • Attend, provide to staff and complete all mandated annual training requirements as defined by JRI policy and referral/funding/accreditation agencies.
  • Other duties as assigned.

Requirements

  • Must be 21 years or older.
  • Must have a Bachelor's Degree or equivalent experience, human services, psychology, education, social work, or a related field from an accredited college or university preferred.
  • Minimum of three years’ experience with target population served and administrative responsibilities.
  • Experience working directly with LGBTQIA+ people, people who use drugs, and high-risk populations preferred.
  • Strong documentation and reporting skills required.
  • Must demonstrate strong team leadership and communication skills.
  • Excellent organizational skills.
  • Demonstrated capacity to collaborate with large teams on multiple projects and respond to a dynamic workflow.
  • Must be able to work independently.
  • Bilingual candidates (Spanish / Portuguese) strongly encouraged to apply.
  • Applicants must maintain a valid driver’s license and have access to an insured vehicle as travel is required.

Pay

The pay range for this position is $54,000 to $59,000 per year. This is a contract-funded position, meaning the pay is contingent on contract funds available at the time of hire. The listed range reflects what we reasonably and in good faith expect to pay for this role at the time of posting the position. The actual compensation an employee is offered may vary based on several factors such as experience, education, licensure, certifications and qualifications.

Schedule

This is a full-time, 40 hours per week position. It is also a hybrid position.
