Overview
Samaritan Daytop Village is a nationally recognized comprehensive Health and Human Services Agency serving over 33,000 New Yorkers annually across New York City and the greater New York Area. This non-profit agency seeks a Senior Case Manager who can work anywhere.
Responsibilities
- Coordinates, guides, and supervises activities of direct reporting staff.
- Provides direct supervision of Case Managers.
- Communicates all aspects of contract requirements to staff.
- Led and participates in administrative and staff meetings as required.
- Troubleshoots problems and makes decisions in accordance with program policies, procedures, and protocols.
- Makes sure clients' progress towards their goals is documented via progress notes.
- Conducts periodic reviews of all case documentation to ensure accuracy, completeness, and clarity.
- Conducts chart auditing of client files.
- Manages a case portfolio of approximately 25 clients.
- Participates in monitoring the therapeutic environment to ensure behavioral guidelines are followed, a therapeutic milieu is maintained, and interpersonal relationships remain positive.
- Provides individual counseling and facilitates caseload group meetings and other assigned groups.
- Provides psycho-education and counseling relevant to chemical dependency and relapse prevention.
- Administers appropriate behavioral interventions as needed.
- Participates in assessment screenings to determine client needs and preferences; formulates and implements treatment, transition, and discharge/continuing care plans collaboratively with individual clients.
- Attends case conferences and other meetings as scheduled.
Qualifications
- Someone with a High School Diploma or Equivalent and five (5) years of experience preferably supervising Case Managers within transitional/shelter programs or related setting.
- Or an Associate's Degree in Human Services or related field with at least two (2) years of experience supervising Case Managers within transitional/shelter programs or related settings.
- Or a Bachelor’s Degree in Human Services with at least one (1) year of experience supervising Case Managers within transitional/shelter programs or related settings.
- Working knowledge of substance abuse treatment modalities and client self-help/support modalities.
- Available to work a flexible schedule as needed in response to participant and staff needs.
- Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
