Responsibilities
- Coordinate the scheduling and ongoing management of home care services.
- Match clients with appropriate caregivers based on home care needs, availability, and skill set.
- Communicate regularly with clients, families, caregivers, and clinical staff.
- Monitor service delivery, address issues, and ensure continuity of care.
- Maintain accurate documentation and ensure compliance with state and agency regulations.
- Respond to client concerns and resolve service-related issues promptly.
- Participate in quality assurance and performance improvement activities.
- Coordinate with patients, families, and insurance contacts to ensure services are scheduled and plans or changes are communicated promptly.
- Manage scheduling of a patient caseload and associated responsibilities, such as pre-billing, attendance, and visit verification.
- Accurate and timely documentation in patients' and caregivers' records.
- Work occasionally on weekends and holidays as needed.
- Identify and escalate issues to supervisors.
- Perform other duties as assigned.
Requirements
- Strong communication skills, both written and verbal.
- Excellent customer service skills.
- Strong time management and organization skills.
- Adaptability and flexibility.
- Proficient in Microsoft 365.
- HHAX experience preferred.
- 1 to 3 years' experience as a Home Care Coordinator.
- Ability to multitask.
Desired Skills and Experiences
- Home health care experience.
- Experience with HHAeXchange Homecare Application.
- Call center customer service representative experience.
- Proficiency in Microsoft Office 365 applications: Teams, Outlook, Excel, Word.
Pay
$21.00–$25.00 per hour
