About the role

Seeking a licensed Occupational Therapist to deliver in-home occupational therapy services, focusing on assessments, planning, and treatment to help patients achieve independence.

Responsibilities

• Conducting initial and ongoing evaluations
• Interpreting test results
• Developing and modifying plans of care
• Documenting progress to ensure optimal outcomes
• Discharge planning
• Patient education
• Collaborating with healthcare teams
• Supervising related personnel when applicable
• Completing required assessments and maintaining compliance with policies

Qualifications & Skills

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license (e.g., WI, MN)
• Valid CPR and driver's license

Key Duties

• Perform assessments evaluating mobility and function
• Create and update individualized treatment plans
• Deliver therapy services per physician orders
• Maintain thorough documentation and modify care as needed
• Coordinate with healthcare providers and educate patients/caregivers
• Supervise other staff (OTAs, HHAs) as appropriate
