Position Overview

An Occupational Therapist in home health provides personalized, one-on-one care to help patients live independently and safely in their homes.

Responsibilities

• Conducting initial and ongoing assessments of mobility, strength, and function
• Developing comprehensive plans of care with clear goals and expected outcomes
• Delivering occupational therapy services per physician orders
• Documenting progress and modifying treatment plans as needed
• Collaborating with healthcare team members
• Educating patients and caregivers
• Participating in discharge planning
• Supervising other personnel
• Completing OASIS assessments

Requirements

• A degree in Occupational Therapy from an accredited program
• A current state license
• A valid CPR
• A driver’s license
