Responsibilities

• Performing initial and ongoing assessments of patient mobility, strength, and function.
• Developing and documenting personalized care plans with measurable goals and outcomes.
• Delivering therapy services in accordance with physician orders and modifying plans as needed.
• Maintaining accurate documentation to ensure compliance and optimize reimbursement.
• Collaborating with interdisciplinary teams, patients, and caregivers to coordinate care and discharge planning.
• Supervising auxiliary personnel where applicable and completing assessments like OASIS within scope of practice.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid, unrestricted state license as an OT.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
