About the Role

Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions as directed by physicians within home health care. The role involves performing initial and ongoing assessments of patients' mobility, coordination, and strength, and developing personalized plans of care with measurable goals and outcomes.

Responsibilities

• Conduct assessments to evaluate patient functional status.
• Develop and update individualized treatment plans.
• Deliver therapy services per physician orders.
• Maintain timely and accurate documentation.
• Coordinate services with healthcare team and educate patients and caregivers.
• Supervise other clinical personnel (e.g., OT Assistants and Home Health Aides) as applicable.
• Complete assessments like OASIS where allowed by regulation.
• Discharge planning and collaboration with healthcare team members.

Requirements

• Degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an OT.
• Valid CPR certification.
