Position Overview
An Occupational Therapist in home health provides one-on-one, flexible scheduling care to help patients live independently.
Key Responsibilities
- Performing assessments of mobility, strength, and function
- Developing and implementing personalized care plans
- Documenting progress
- Collaborating with healthcare teams and family members
- Discharge planning and education
- Supervising other personnel like OTA and HHA
- Ensuring documentation compliance
- Participating in service coordination
- Completing assessments such as OASIS where permitted
Qualifications & Skills
- Degree in Occupational Therapy from an accredited program
- Current, unrestricted state license (WI and MN)
- Proficiency in assessments, documentation, and policy compliance
- Valid CPR and driver's license
- Preferred: Home health therapy experience and OASIS documentation familiarity
