RN Wound Care Coordinator Ft days

Job Summary

Kindred Hospital San Gabriel Valley is a 76-bed long-term acute care hospital offering specialized care for extended recovery periods. The position involves managing the facility’s wound care program, providing direct patient care, and consulting with staff and physicians.

Essential Functions

• Develops and implements the facility wound care program in conjunction with national standards.
• Establishes standards of care, competencies, policies, and procedures for wound care.
• Consults with Wound Care Clinicians and acts as a consultant to facility staff.
• Provides ongoing education to staff on wound care products.
• Evaluates wounds and develops treatment plans.
• Assists in developing comprehensive wound management plans and wound prevention strategies.
• Reviews medical records and ensures proper documentation and treatment.
• Investigates adverse events related to wounds and develops action plans.
• Participates in clinical outcome monitoring and performance improvement initiatives.
• Maintains a cost-effective wound care formulary and collaborates with Support Center Clinical Operations.
• Trains new hire Wound Care Coordinators.

Pay Range

$47.00 - $59.00/Hr

Education

Degree from an accredited nursing program.

Licenses/Certifications

• Current state RN license; BSN preferred.
• Professional certification in WOCN, CWS, or WCC within 12 months of employment.
• BLS required.

Experience

1-3 years licensed professional nursing experience with previous acute care nurse experience.
