Role Overview
As a Registered Nurse, you will deliver and coordinate home-based nursing care in compliance with laws, regulations, and policies. Your responsibilities include conducting clinical assessments, developing personalized care plans, ensuring patient eligibility, and collaborating with healthcare professionals, patients, families, and community resources to maintain continuity of care.
Key Responsibilities
- Provide high-quality clinical services within scope and infection control standards
- Coordinate care from admission to discharge with the care team
- Complete assessments aligned with federal, state, and payer requirements
- Verify patient eligibility and medical necessity for services
- Develop and adjust individualized care plans with community providers
- Ensure care plans meet patient needs, revising with physician approval
- Operate effectively in diverse home environments regardless of client background
Qualifications & Skills
- Unrestricted RN licensure in the practice state
- Valid driver's license, vehicle insurance, and reliable transportation
- Preferred: CPR certification, 1+ years RN experience, independent work ability, strong communication, writing, and organizational skills
Compensation & Benefits
Hourly pay ranges from $28.94 to $51.63, depending on experience and location.
