Role Overview
The Registered Nurse provides and directs nursing care in patients' homes, ensuring compliance with laws, regulations, and agency policies. They coordinate care plans with healthcare professionals, patients, families, and community resources to maintain continuity of care.
Key Responsibilities
- Deliver high-quality clinical services within scope of practice and infection control standards
- Coordinate care from admission to discharge with the care team
- Conduct clinical nursing assessments in line with federal/state requirements and payer needs
- Verify patient eligibility and medical necessity for services
- Create and update individualized care and service plans collaboratively with community providers
- Adjust care plans as needed with physician approval
Qualifications & Skills
- Current, unrestricted RN licensure in the practice state
- Reliable transportation, including valid driver’s license and vehicle insurance or access to public transit
- Current CPR certification
- Ability to work effectively in diverse home environments
Preferred Attributes
- Over one year of RN experience
- Independent work skills
- Strong communication, organizational, and writing abilities
