Role Overview

The Registered Nurse provides and oversees home-based nursing care, collaborating with healthcare professionals to ensure continuity of care. They assess patients, develop individualized care plans, and coordinate services to meet patient needs while ensuring compliance with laws and regulations.

Key Responsibilities

• Deliver high-quality clinical nursing services within scope of practice and infection control standards
• Coordinate care with team members from admission to discharge
• Complete assessments as per federal/state requirements and payer needs
• Verify patient eligibility and medical necessity for services
• Develop and update personalized care plans in collaboration with community providers
• Ensure care plans meet patient needs, with revisions approved by physicians

Qualifications & Skills

• Current, unrestricted RN licensure in the practice state
• Valid driver's license, vehicle insurance, and access to reliable transportation or public transit
• CPR certification
• Ability to adapt to diverse home environments regardless of client background
• Preferred: 1+ years of RN experience, independence, strong communication, organizational, and writing skills
