RN Visit Nurse

Role Overview

As a Registered Nurse, you will deliver and coordinate in-home nursing care in compliance with laws, regulations, and policies. Your responsibilities include assessing patients, developing and updating individualized care plans, and collaborating with healthcare professionals, patients, families, and community resources to ensure continuity of care.

Key Duties

• Provide high-quality clinical services within scope of practice and infection control standards
• Coordinate care from admission to discharge with the care team
• Complete clinical assessments per program and payer requirements
• Verify patient eligibility and medical necessity for services
• Develop and revise care plans with community providers
• Ensure care plans meet patient needs, with physician approval for revisions

Qualifications & Skills

• Unrestricted RN licensure in the practice state
• Valid driver's license, vehicle insurance, and access to transportation
• Ability to work effectively in diverse home environments
• Preferred: 1+ years of RN experience, independence, strong communication, organization, and writing skills

Compensation & Benefits

• Pay ranges from $38.85 to $53.75 per hour, based on experience and market factors
