RN Triage - Medical Triage

Role: RN Triage for Medical Group Care Team, focused on patient assessment and care coordination. Responsibilities: Review patient messages, conduct telephone assessments, determine urgency, provide medical advice, and direct patients to appropriate care levels. Evaluate patient understanding, document interactions using EMR, and collaborate with clinical staff. Handle high-risk cases promptly, arrange emergency services when needed, and ensure excellent customer service. Participate in quality initiatives and care coordination activities.Qualifications & Skills: Current Illinois RN licensure, CPR certification, minimum three years of RN experience, preferably with telephone triage. Office RN and BSN are preferred. Ability to handle high-stress situations, communicate effectively, and perform physical activities like standing, lifting, and reaching. Proficiency in English, both written and verbal. Key Duties: Patient communication via phone and electronic messaging, clinical decision-making, care planning, and teamwork to support quality patient care and adherence to organizational policies.
