Responsibilities
- Participates in staff meetings, department meetings, team meetings, in-services, and other related activities.
- Attends educational and developmental seminars and meetings as appropriate.
- Utilizes clinical registered nurse assessment and evaluation skills to assess patient needs, prioritize requirements, and determine necessary actions.
- Captures and coordinates urgent patient visits, including providing care, evaluations, medication, and medical supplies.
- Initiates Quality Service Issues Report (QSIR), Incident Reports, and Level of Care (LOC) forms per Hospice of the Comforter policy.
- Provides nursing services to patients and families via telephone support.
- Receives and responds to telephone calls, virtual calls, and emails from patients, caregivers, doctors, or others regarding patient needs or changes in condition.
- Communicates pertinent patient and employee information to interdisciplinary teams and patient care managers daily.
- Consults with manager or clinical leadership regarding unusual situations and potential risk management issues before initiating patient status.
- Affirms the need for other hospice services or community resources and notifies appropriate parties to activate services.
- Documents patient contact and activity into electronic medical records and maintains accurate logs and dispatch records.
Qualifications
- Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
- Knowledge of electronic medical record.
- Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee.
- Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software.
- Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form.
- Ability to follow standard precaution guidelines for infection control, including when providing patient care.
- Ability to complete AdventHealth Hospice Care CFL Orientation and Training, as necessary.
- Education: Associate's of Nursing [Required], Bachelor's of Nursing [Preferred]
- Field Of Study: N/A
- Work Experience: 2+ years experience as a registered nurse [Required], Hospice or home health, acute care nursing experience [Preferred]
