RN - Transfer Center Coordinator | PRN

The RN coordinator at Avera Palm Place is responsible for facilitating the efficient and seamless transfer of patients to the most appropriate Avera hospitals within the system. This role involves collaborating with various departments and healthcare providers to ensure timely and appropriate patient placement.

Responsibilities

• Oversees the flow of patients from outlying facilities and clinics to ensure an efficient entry into the facility.
• Prioritizes and places patients in the appropriate regional sites based on critical thinking and adherence to department policies and guidelines.
• Works as a liaison with the bed placement coordinator to ensure timely transfer of patients.
• Evaluates the transfer center process to achieve desired outcomes.
• Collaborates with physicians, nurses, and healthcare providers to assess patient placement.
• Utilizes research and evidence-based findings in practice when appropriate.
• Maintains awareness of healthcare technology and information management systems to support patient care.

Requirements

• The individual must be able to work the specified hours.
• Must have an active license in the state of practice.
• Requires 1-3 years of nursing experience.

Qualifications

• Visual acuity adequate to perform position duties.
• Effective communication skills.

Pay
