About the Role
Responsible and accountable for the independent planning, organization, implementation, and evaluation of Ambulatory Specialty Practice patients. Ensure adherence to hospital and departmental policies and procedures. Patient care assignments may include neonate, pediatric, adolescent, adult, and geriatric age groups.
Responsibilities
- Perform all RN Nursing duties and assist in specialty clinic; triage patients as necessary in daily walk-in clinic.
- Coordinate all activities of the specialty related to patient care.
- Serve as liaison between patient/family and various members of the health care team.
- Counsel and teach families regarding medical conditions, diagnostic procedures, medication control, and other interventions.
- Coordinate and facilitate the delivery of services to provide optimal care for the patient within the specialty patient area, including specialty surgical procedures.
- Collaborate with other members of the specialty team about the candidacy of potential patients.
- Participate in programs and committees related to the specialty or as directed.
- Conduct professional and public speaking engagements for educational and public relations purposes.
- Participate in educational opportunities and workshops associated with the specialty.
- Provide statistical reports as needed.
- Rotate on-call duties as directed, including holidays, nights, and weekends.
- Review daily lab and diagnostic work on specialty evaluations to assist in directing the specialty evaluation.
- Direct and coordinate educational activities for hospital employees regarding specialty surgeries; assist physicians with research projects related to specialty surgeries.
- Collaborate with community physicians on the daily care of specialty patients.
- Coordinate diagnostic procedures for complications that occur after specialty surgery in consultation with physicians.
- Direct the management of patient care caseload pre- and post-operative to provide continuity for the specialty patient and their families.
- Maintain established departmental policies and procedures, objectives, and quality assurance programs.
- Provide input and assist in the formulation of departmental policies, procedures, and budget as requested.
- Follow patient safety-related policies, procedures, and protocols.
- Demonstrate a proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes.
- Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk.
- Report potential or actual patient safety concerns, medical errors, and/or near misses in a timely manner.
- Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations they don’t understand or may "not seem right."
- Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable.
Qualifications
- Education:
- Essential: Graduate of a nationally accredited nursing program.
- Nonessential: Bachelor’s Degree in Nursing.
- Experience:
- Essential: 3 years of directly related experience.
- Nonessential: Bilingual in English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo.
- Credentials:
- Essential: RN license in New Mexico or as allowed by reciprocal agreement; CPR for Healthcare/BLS Provider or Professional Rescuers within 30 days of hire.
- Physical Conditions:
- Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
- Working conditions: Significant exposure to chemicals and biohazardous materials; PPE required. May work rotating shifts, holidays, and weekends.
- Tuberculosis testing completed upon hire and as required thereafter.
