About the role

HCN is seeking a highly skilled, experienced, and accountable RN Supervisor to build a long-term employment relationship with our agency, clients, staff, and care team partners under the guidance of our Northeast Ohio RN Program Administrator. This full-time leadership position manages care across approximately 27 counties in Northeast Ohio, with a central focus in Summit, Stark, and Tuscarawas counties.

Responsibilities

  • Perform case management, starts of care (SOC), admissions, recertifications, transitional care, resumptions of care (ROC), clinical reviews, and other assessments and services
  • Conduct new hire orientations, in-service and supervisory reviews, and contribute to training and education initiatives
  • Influence care practices, outcomes, and the success of clients, employees, and the business overall
  • Coordinate, re-evaluate, and confirm medical necessity of continued home health care services
  • Provide patient care and ensure optimal outcomes through managed care, patient assessments, compliance, documentation, and discharge planning
  • Submit updated documentation of patient conditions and ongoing care needs in compliance with state, federal, and agency guidelines
  • Ensure successful communication among physicians, agency staff, service administrators, caregivers, clinicians, and clients/families
  • Assess clients while directing, planning, and initiating appropriate actions in home care situations
  • Manage and supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides
  • Document, communicate, and coordinate patient status and changes in condition to the care team
  • Promote personal safety and a safe environment for clients and coworkers

Requirements

  • At least 1 year of experience in nursing supervision
  • At least 1 year of paid experience at/with a home health agency or in-home care provider (or equivalent); 2–5 years preferred
  • Experience with wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership
  • Graduate of an accredited School of Nursing
  • Minimum of 1 year experience as a Registered Nurse with an active RN license in the State of Ohio
  • CPR and First Aid certifications (or ability to obtain prior to hire)
  • Strong background in nursing, patient care, and confidentiality
  • Exceptional on-time and attendance record with a consistent, reliable approach to work
  • Desirable traits: reliable, honest, professional, compassionate, detail-oriented, accountable, friendly, respectful, and positive
  • Familiarity with HIPAA regulations and medical terminology
  • Willingness and ability to perform required physical demands
  • Consent and ability to pass a State & Federal background check (including fingerprints)
  • 18 years or older with an active driver’s license/State ID and U.S. work authorization
  • Reliable transportation to all scheduled visits within a 2-hour radius

Schedule

  • Full-time, salaried position with weekday daytime availability (Monday–Friday, 8:00 a.m.–5:00 p.m.)
  • Occasional evening or weekend work
  • Regular regional travel required across approximately 27 counties in Northeast Ohio (up to a 2-hour radius)
  • Door-to-door mileage reimbursement provided
  • No on-call or overnight travel requirements

Pay

  • Full-time annual salary (plus PTO and mileage): $80,000–$85,000 per year, based on final employment and travel agreements and minimum visits/services/recertifications required per week
  • Full-time hourly wage (plus mileage): $35–$40/hour
