RN Supervisor - Home Health Agency (Full-Time, Salaried NE Ohio Travel)
About the role
HCN is seeking a highly skilled, experienced, and accountable RN Supervisor to build a long-term employment relationship with our agency, clients, staff, and care team partners under the guidance of our Northeast Ohio RN Program Administrator. This is a full-time leadership position managing care across approximately 27 counties in Northeast Ohio, with a central focus in Summit, Stark, and Tuscarawas counties.
The role blends nurse supervisory services with recertification visits and travel as needed to support clients and staff throughout Northeast Ohio. The RN Supervisor works closely with and reports directly to the Northeast Ohio RN Program Administrator (local) and the Director of Clinical & Administrative Operations (statewide).
For those pursuing career growth, a defined path for advancement is supported into roles including RNCM, Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA/Compliance leadership, and other positions.
Responsibilities
- Case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC)
- Clinical reviews and other assessments and services
- New hire orientations, in-service and supervisory reviews
- Input to influence training and education, care practices and outcomes, and the success of clients, employees, and the business
- Perform client assessments while directing, planning, and initiating appropriate actions in home care situations
- Manage and supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides
- Document, communicate, and coordinate patient status and changes in condition to the care team
- Perform job duties while promoting personal safety and a safe environment for clients and coworkers
Requirements
- Graduate of an accredited School of Nursing
- Minimum of 1 year of experience as a Registered Nurse with an active RN license or ability to practice in the State of Ohio
- At least 1 year of experience in nursing supervision
- At least 1 year of paid experience at or with a home health agency or in-home care provider (or equivalent); 2–5 years preferred
- Experience in wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and non-OASIS assessments, and excellent documentation and field staff leadership
- CPR and First Aid certifications required (or obtainable prior to hire through the agency or accredited third parties)
- Strong background in nursing, patient care, and confidentiality
- Exceptional on-time and attendance record with a consistent and reliable approach
- Familiarity with HIPAA regulations and medical terminology
- Willingness and ability to perform required physical demands
- Consent and ability to pass a state and federal background check (including fingerprints)
- 18 years or older with an active driver’s license/state ID and U.S. work authorization
- Reliable transportation to all scheduled visits within the full travel radius and assigned clients (up to at least 2 hours required)

Schedule
- Full-time, salaried position with weekday daytime availability (Monday–Friday, 8:00 AM–5:00 PM)
- Occasional evening or weekend work
- Regular regional travel required across approximately 27 counties in Northeast Ohio (up to a 2-hour radius)
- Typical travel distances include a 60-minute radius daily with up to 2 hours on occasion
- Door-to-door mileage reimbursement provided
- No on-call requirements and no overnight travel required

Pay
- Full-time annual salary (plus PTO and mileage) starts between $80,000 and $85,000 per year, based on final employment and travel agreements and pre-determined minimum visits/services/recertifications required per week
- Full-time hourly wage (plus mileage) option: $35–$40/hour
