Responsibilities

  • Coordinates the evaluation process of the defined patient population.
  • Serves as a liaison between the referring provider's office and Vanderbilt provider.
  • Develops and manages the processes related to pre-admission and post-discharge care transitions.
  • Assists in the development and dissemination of patient education materials/information.
  • Assists with discharge/transition planning in collaboration with the multi-disciplinary team.
  • Demonstrates reflective practice by constantly evaluating care coordination and supports the development of protocols for practice based on evidence.

Qualifications

  • Minimum of 5 years of clinical RN experience
  • Spine or neurology-related experience preferred
  • Prior experience with Joint Commission (JCAHO) standards and survey readiness required

Skills & Competencies

  • Strong project management and organizational skills
  • Ability to manage, analyze, and present data effectively (including dashboards and reporting tools)
  • Demonstrated ability to lead cross-functional collaboration and drive outcomes
  • Excellent communication, facilitation, and relationship-building skills
  • Detail-oriented with a focus on quality, compliance, and continuous improvement
