Responsibilities
- Coordinates the evaluation process of the defined patient population.
- Serves as a liaison between the referring provider's office and Vanderbilt provider.
- Develops and manages the processes related to pre-admission and post-discharge care transitions.
- Assists in the development and dissemination of patient education materials/information.
- Assists with discharge/transition planning in collaboration with the multi-disciplinary team.
- Demonstrates reflective practice by constantly evaluating care coordination and supports the development of protocols for practice based on evidence.
Qualifications
- Minimum of 5 years of clinical RN experience
- Spine or neurology-related experience preferred
- Prior experience with Joint Commission (JCAHO) standards and survey readiness required
Skills & Competencies
- Strong project management and organizational skills
- Ability to manage, analyze, and present data effectively (including dashboards and reporting tools)
- Demonstrated ability to lead cross-functional collaboration and drive outcomes
- Excellent communication, facilitation, and relationship-building skills
- Detail-oriented with a focus on quality, compliance, and continuous improvement
