RN Patient Care Coordinator, Spine Center

About the role

Vanderbilt Health is seeking a dedicated Registered Nurse to join our Spine Clinic team. The ideal candidate will assist in developing and meeting key Pillar outcomes and system improvement goals, focusing on improving patient outcomes by reducing all cause hospital readmissions and coordinating care episodes.

Responsibilities

• Coordinates the evaluation process of the defined patient population.
• Serves as a liaison between the referring provider's office and Vanderbilt provider to support patient access.
• Develops and manages processes related to pre-admission and post-discharge care transitions.
• Assists in the development and dissemination of patient education materials.
• Coordinates handovers including outpatient care coordinators, home health care nurses, cardiac rehab, skilled nursing facilities, etc.
• Demonstrates reflective practice by evaluating care coordination and supporting the development of protocols based on evidence.

Requirements

• LIC-Registered Nurse - Licensure
• Relevant Work Experience: 5 years
• Bachelor's Degree in Nursing or related field
• Evidence-Based Practice (Advanced)
• RN Access Patient Education (Advanced)
• Nursing Patient Assessment & Evaluation (Advanced)
• Trend Analysis (Intermediate)

Pay

The starting salary range for this position is $60,000 - $70,000 annually.

Schedule

This position is full-time, Monday through Friday.
