About the role

Aster Creek Nursing and Rehabilitation Center is seeking Registered Nurses (RNs) to plan and deliver direct and indirect patient care through nursing assessments, planning, intervention, and evaluation. RNs develop nursing care plans in coordination with patients, families, and staff, communicate changes in care requirements with physicians and nursing directors, and participate in the discharge planning process.

Responsibilities

• Perform nursing assessments, planning, interventions, and evaluations
• Develop and update nursing care plans in collaboration with patients, families, and staff
• Communicate changes in patient care requirements to physicians, nursing directors, and co-workers
• Participate in the discharge planning process

Requirements

• Active Registered Nurse (RN) license in the state of New Jersey
• Long-term care experience is a plus
