RN - Referral Intake Nurse (Per Diem)
About the role
The Referral Intake Nurse is responsible for the initiation, screening and triage of all Home Health and/or Hospice referrals. This position is responsible for maintaining relationships with referral sources and insurance companies. Additionally, the Referral Intake Nurse is responsible for the education of referral sources as to types of patients served and eligibility requirements for service. This position is responsible for oversight/review of all referrals taken by Intake Support Specialist (ISS).
Primary duties
- Receives and documents Home Health and/or Hospice, Palliative Care referrals called or faxed to the department from all outside sources, including hospitals, clinics, other facilities or families.
- Assesses clinical needs of patients interpreting and utilizing data obtained from referral sources and medical records as available.
- Determines patient eligibility for Home Health and/or Hospice, Palliative Care services as measured against program criteria.
- Verifies skilled care orders with physicians as needed, obtains history and physical from physician as needed.
- Obtains insurance information from referral source and forwards information to Business Office insurance verifier.
- Obtains insurance authorization for initial visits when appropriate and required by insurer.
Required qualifications
- Current State of Washington professional nursing license.
- Current BLS Healthcare Provider certification by date of hire.
- 2 years of Home Health, Hospice or other Home Care experience.
- Prior marketing, sales and/or community outreach experience.
- Current Washington State Driver's license and proof of insurability, if the position requires regular driving.
- Working knowledge of Medicare COP, JC Standard.
- Familiarity with Third party insurance and Medicare HMO's.
Preferred qualifications
- Master's degree in nursing.
Pay
$48.21 - $87.11 per hour. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.

This position is covered by a collective bargaining agreement between EvergreenHealth and the Washington State Nurses Association (WSNA). Union membership is optional.
