Intake Nurse-Home Care-Per Diem

Responsibilities

• Assess patient eligibility for the specific business line by reviewing clinical information and obtaining necessary information to support clean billing.
• Ensure all referrals are clinically complete, efficient, and accurate.
• Communicate effectively with physician offices and external facilities, demonstrating customer service skills.
• Identify opportunities to assist with custom needs related to home care services.
• Support growth of all product lines continuously.
• Process all referrals received from various sources (referral systems, faxes, phone).
• Utilize clinical judgment to ensure referrals are clinically appropriate and completed efficiently and accurately.
• Ensure open communication with operations.
• Use a tracking system to follow up on pending cases.
• Solve problems with staff on pertinent patient referral issues.
• Maintain current knowledge of Medicare reimbursement criteria and insurance contracts for all at-home product lines.
• Awareness of patients' payment responsibilities.
• Ensure face-to-face completion and communication with operations.
• Act as a liaison between physician offices, external facilities, and hospitals, demonstrating customer service skills.
• Verify clinical documentation to support home health referrals.
• Receive verbal orders for home health orders when required.
• Provide information to referral sources regarding available programs and services.
• Remain knowledgeable in all areas of home care services and the resources available to referral sources in the community.
• Work with team leaders to assist when help is needed.
• Address clinical issues with supervisor, manager, and staff when needed.

Qualifications

• Two (2) years health care experience in acute care, skilled nursing facility, rehabilitation, or home health within the last 5 years.
• Proficient in Google platform (Google Docs, Google Sheets).
• Education: Bachelor of Science in Nursing (BSN) preferred.
• Experience: Homecare experience preferred.
• Licenses and Certifications: NJ State Professional Registered Nurse License or Licensed Practical Nurse required.
