Overview
We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality healthcare services to patients in their homes. The ideal candidate will possess a strong clinical background and demonstrate excellent patient assessment skills. This role is essential in ensuring that patients receive the necessary care and support in a comfortable environment, promoting their overall well-being and recovery.
Job Description
- Evaluate client’s in their homes under the direction of the DPS to assess the client’s homecare needs, create or adjust a Plan of care, and oversee that the caregiver is implementing the plan of care appropriately.
- Instruct and advise patients and/or families as well as the home health aides in their care.
- Reassess clients in the beginning of each month and schedule visits according to both her and the client’s availability.
- Assign a list of aide annual supervisions at the beginning of the month.
- Conduct start of care assessments within 24 hours of the start of care.
- Conduct PRN visits if there is a change of condition requiring a nurse's follow-up.
- Conduct in-office Orientations to all new caregivers in a group setting on a predetermined schedule at our Newburg Office.
- Submit all documentation from visits to the DPS within 24 hours of the visit.
- Communicate to the DPS any concerns or client changes.
- Occasionally conduct visits in Rockland County when the Rockland nurses are out.
Requirements
- Valid and current New York State RN license and registration.
- Active driver’s license and reliable transportation.
- Clear English.
- Strong assessment skills.
- Able to work independently and as part of a team.
- Good oral and written communications skills.
- Previous homecare or assessment nursing experience preferred.
Qualifications
- Updated physical exam.
- Up to date with all immunizations.
- PPD.
