About the role

As The Intake RN/Home Care Coordinator, You Will Be responsible for coordinating home care services for participants. Make initial visits and subsequent home safety assessments. Complete ongoing evaluations of participant needs, supplies, education, and ongoing quality of care. Provide nursing visits in the home for wound care, foley management, disease education, etc.

Responsibilities

• Schedule: pick up to cover open shifts on weekdays
• Make initial visits and subsequent home safety assessments

• Provide nursing visits in the home for wound care, foley management, disease education, etc.

Requirements

• Licensure as an RN in the state of Michigan
• Prior experience working with a frail or elderly population
• Prior home care experience
• The ability to multi-task, work independently as well as in a team setting, and have a passion for those whom we serve through this amazing program

Qualifications

• Eligible for the PACE program (55+ years old, Kent County resident, able to live safely at home, and meeting the nursing facility level of care)

Pay

Compensation is commensurate with experience.

Schedule

Shifts are weekdays, 8am - 4:30pm.
