About the role

VNA nurses make a difference! Home Health is a specialized setting of care that’s incomparable to any other, providing a unique insight into those factors that can influence how patients manage their health conditions at home. We are highly skilled nurses—discovering home safety deficits, nutrition and hydration concerns, potential medication errors, and assessing the impact of social determinants of health.

Responsibilities

• Comprehensive patient assessment
• Medication reconciliation and education
• Disease process education
• Wound and ostomy care
• Home infusion therapy
• Skilled nursing interventions such as pleural drainage, urinary catheter management, etc.

Requirements

Work Schedule: Per Diem—Every other weekend commitment, holiday and on call.
