Intake Nurse-Home Care-Per Diem

Responsibilities

• Assess patient eligibility for the specific business line
• Ensure all referrals are clinically complete, efficient, and accurate
• Communicate effectively with physician offices and external facilities
• Identify opportunities to assist with custom needs for home care services
• Support growth of all product lines continuously
• Process all referrals received from various sources
• Utilize clinical judgment to ensure referrals are clinically appropriate and completed efficiently and accurately
• Ensure timely communication and appropriate follow-up on pending cases
• Problem solve with staff on patient referral issues
• Maintain current knowledge of Medicare reimbursement criteria and insurance contracts for all at-home product lines
• Awareness of patients' payment responsibilities
• Ensure face-to-face completion and communication to operations
• Act as a liaison to physician offices, external facilities, and hospitals
• Provide information to referral sources regarding available programs and services
• Remain knowledgeable in all areas of home care services and the resources available to referral sources in the community
• Work with team leaders to assist when help is needed
• Address clinical issues with supervisor, manager, and staff when needed

Qualifications

• Two (2) years health care experience in acute care, skilled nursing facility, rehabilitation, or home health within the last 5 years
• Proficient in Google platform (Google Docs, Google Sheets)
• Education, Knowledge, Skills and Abilities Required: Two (2) years health care experience in acute care, skilled nursing facility, rehabilitation, or home health within the last 5 years
• Education, Knowledge, Skills And Abilities Preferred: Bachelor of Science in Nursing (BSN), Homecare experience
• Licenses And Certifications Required: NJ State Professional Registered Nurse License or Licensed Practical Nurse, HMH is committed to pay equity and transparency for our team members.
