What will you be doing
We are seeking an experienced, compassionate, and detail-oriented Intake Registered Nurse or Licensed Practical Nurse to join our team on a per diem basis. This position plays a key role in ensuring patients receive timely, appropriate home health services and offers remote flexibility following the completion of onsite training. Remote work must be done in Massachusetts.
Schedule
- Minimum commitment of 2 shifts per month
- Department operational hours are 7 days per week, with day shifts between 8:00 AM and 5:30 PM
Compensation
Ranges from $38.00/hr. - $48.00/hr. based on licensure and experience
In this role, you will
- Review and clinically assess incoming referrals to determine appropriateness for home health services.
- Ensure referrals are processed accurately and in a timely manner while maintaining compliance with agency, Medicare, Managed Care, and regulatory requirements.
- Collaborate with Clinical Managers to coordinate daily admissions, prioritize referrals, and manage agency capacity.
- Partner with Intake Coordinators to oversee the referral process from receipt through admission.
- Communicate with physicians, hospitals, case managers, and other referral sources to obtain clinical documentation and facilitate timely admissions.
- Build and maintain strong relationships with referral partners by providing education, support, and exceptional customer service.
- Serve as a clinical resource for the Intake team, utilizing sound clinical judgment to prioritize and triage referrals.
- Maintain accurate documentation within the electronic medical record (EMR).
What we're looking for in you
- Current Massachusetts RN or LPN license in good standing.
- Minimum of 3 years of nursing experience required.
- 1+ year of direct home health experience preferred.
- Medicare and Managed Care experience required.
- Working knowledge of home health and hospice regulations.
- Strong clinical assessment, critical thinking, and triage skills.
- Excellent communication, interpersonal, and relationship-building abilities.
- Proven ability to prioritize multiple referrals in a fast-paced environment.
- Strong organizational skills with exceptional attention to detail.
- Experience with electronic medical records; MatrixCare experience is preferred.
- Ability to work independently in a remote environment while collaborating effectively with an interdisciplinary team.
