RN Patient Navigator

Status: Full time, benefits eligible, 40 hours per week. Schedule varies; includes Monday-Friday with rotating call.

About the Role

Guides patients through the healthcare system, establishing contact with patients, family members, and/or caregivers. Understands the clinical pathways established by the clinical specialty and directs patients to healthcare services in the organization and in the community for timely diagnosis, treatment, and survivorship. Actively identifies and addresses barriers to care that might keep the patient from receiving timely and appropriate treatment for specific diagnoses. Assists the patient and physicians in the transition from active treatment to continuing healthcare.

Responsibilities

  • Utilize the nursing process to provide patient/family-centered care focusing on physical, emotional, spiritual, cultural, religious, and environmental needs, including assessment, development of nursing diagnoses, outcome identification, planning, implementation of interventions, coordination of care, health teaching and promotion, and evaluation of goals and interventions consistent with policies.
  • Coordinate the effective and efficient delivery of patient care services through the application of care management principles.
  • Identify clinical priorities and initiate action to improve patient outcomes; act as a patient advocate throughout the continuum of care.
  • Document in accordance with departmental reporting standards; collect and maintain up-to-date and accurate program data and documentation.
  • Deliver reports that demonstrate outcomes and performance improvement activities; adhere to established productivity benchmarks.
  • Implement outreach efforts to establish and maintain positive working relationships with key community agencies; serve as community liaison and educator in underserved communities.
  • Demonstrate the ability to function in a professional setting through active participation in a professional practice model, including shared governance, adherence to standards of care, peer review, professional development, leadership, clinical excellence, team skills, autonomous practice, and accountability.
  • Communicate effectively to promote sharing of information throughout the continuum of care and as an effective member of the healthcare team.
  • Responsible for ongoing evaluation and quality improvement of the Navigator Program; track all activity as defined by departmental and program standards.
  • Work closely with community-based service providers to identify and address community needs for services and education regarding prevention, early detection, medications, and first aid.
  • Provide age/developmentally appropriate patient care in accordance with Age-Specific guidelines for the specific age groups served.

Requirements

  • Graduate from an accredited school of nursing; BSN required.
  • Minimum two years' experience as a registered nurse in the specialty required; 3-5 years strongly preferred.
  • Current licensure to practice as a Registered Nurse in North Carolina.
  • Certification as an RN preferred; BCLS required.

  • Expertise in MS Office.
  • Ability to prepare program reports frequently and efficiently.
  • Bilingual preference with fluency in Spanish desirable.

Pay

Pay range: $41.10 - $61.65 per hour. Base compensation listed within the range is based on factors such as qualifications, skills, relevant experience, and/or training. Premium pay such as shift, on-call, and more may be available based on the job.
