Position Responsibilities
- Conduct pre-operative and pre-procedure patient check-ins to review instructions, answer questions, and address concerns about symptoms and expected outcomes.
- Perform post-operative and post-procedure follow-ups to monitor patient recovery, symptom management, and adherence to care plan.
- Oversee the preparation and follow-up processes for patients undergoing surgery or procedures, ensuring all relevant patient information and supplies are in place for both pre- and post-care phases.
- Cook up appropriate home healthcare services are arranged, including nursing visits, physical therapy, and necessary durable medical equipment.
- Assess patients' needs for post-operative supplies (e.g., wound care, mobility aids) and assist in timely procurement.
- Serve as a liaison between patients, families, and the multidisciplinary care team to facilitate communication and continuity of care.
- Participate in quality improvement initiatives and team meetings to optimize patient outcomes and navigation processes.
- Provide patient education to assist with informed decision making, improve timeliness of care, and multidisciplinary care processes.
- Work closely with the department Director, Physicians, and APPs to ensure team goals are met, monitors compliance with established protocols, and reports progress regularly.
- Document patient interactions and care coordination efforts accurately in the electronic medical record system.
- Facilitate timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and to promote continuity of care.
- Manage complex patient cases by providing individualized navigation support and collaborating with supervisors to develop solutions for challenging situations.
- Communicate care strategies and updates to the navigation team, aligning efforts to meet departmental goals and patient care standards.
- Adhere strictly to patient safety and confidentiality standards, including compliance with HIPAA and organizational policies.
- Support organizational unplanned concerns as assigned and report/resolve potential delays of planned surgery.
Qualifications
- Current RN Licensure in State of Florida.
- BLS and CPR required.
- Education: Graduated from an accredited school of nursing. BSN preferred.
- Experience: 3 - 5 years experience in patient/healthcare relations or human services field (e.g. nursing, teaching, counseling) or equivalent combination of education and experience required.
