About the role
We are seeking a Registered Nurse (RN) to provide in-home care for Pediatric and/or Adult patients.
Responsibilities
- Utilize the nursing process to plan, implement, and evaluate patient care.
- Develop and modify care plans based on patient needs and physician input.
- Implement therapeutic, preventive, and rehabilitative nursing actions.
- Respond to patient condition changes and report to physicians or supervisors.
- Administer medications and treatments, monitor and document effectiveness.
- Review care plans and identify education, care coordination, and discharge planning needs.
Qualifications & Skills
- Valid RN license in the practice state.
- Current health certificate, PPD or chest X-ray, and BLS certification.
- Preferably one year of RN experience.
Key Duties
- Provide safe, compassionate patient care in home settings.
- Follow established protocols and revise care plans as needed.
- Educate patients and families about health management and care goals.
- Coordinate care and communicate effectively with healthcare team.
